1. Nature of Telehealth
Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between you and a licensed clinician who is not in the same physical location as you. ELIVE RX LLC facilitates telehealth services that may include asynchronous review through Uniloop (e.g., intake forms and messaging) and, when clinically appropriate, synchronous communication (e.g., video or phone).
2. Benefits of Telehealth
- Improved access to care, especially for patients in remote or underserved areas
- Convenience and reduced travel time
- Efficient follow-up and monitoring for ongoing programs
3. Limitations and Risks
- Telehealth may not be appropriate for all conditions or emergencies
- Technology failures may delay or interrupt care
- Physical examination options may be limited compared to in-person visits
- Information transmitted electronically may, despite safeguards, be intercepted or lost
- Your clinician may determine that you need in-person evaluation and refer you accordingly
4. Not for Emergencies
ELIVE RX telehealth services are not for medical emergencies. If you experience a medical emergency, call 911 or go to the nearest emergency room immediately. Do not rely on telehealth for urgent or life-threatening conditions.
5. Privacy
We use reasonable safeguards to protect the privacy and security of your health information. However, no system is completely secure. You are responsible for using a private location and secure internet connection during telehealth interactions. 6. Technology Requirements
You are responsible for providing the hardware, software, and internet connection needed to use our services. You must provide accurate contact information so our care team can reach you. If you experience technical difficulties, contact jason@eliverx.com.
7. Prescription and Pharmacy Fulfillment
If a licensed clinician determines that medication is appropriate, prescriptions may be sent to a licensed partner pharmacy ([TBD — pharmacy name(s)]) for fulfillment and delivery. A prescription will not be issued or dispensed prior to clinical review and provision of care by a licensed medical professional.
8. Your Rights
- You may withdraw consent for telehealth services at any time by notifying us in writing at jason@eliverx.com
- Withdrawal of consent will not affect your ability to receive care through other means, where available
- You may ask questions about your care, alternatives, and the telehealth process before and during treatment
9. Consent
By completing clinical intake, creating an account, or otherwise using ELIVE RX telehealth services, you acknowledge that:
- You have read and understand this Telehealth Consent
- You have had the opportunity to ask questions
- You consent to receive telehealth services from licensed clinicians through ELIVE RX
- You understand the benefits, limitations, and risks described above
