These Terms of Service ("Terms") are a binding agreement between you and ELIVE RX LLC ("ELIVE RX," "we," "us," or "our"). By accessing our website, creating an account, or purchasing a program, you agree to these Terms and our Privacy Policy, Telehealth Consent, and Refund Policy.
2. Services Description
ELIVE RX provides a telehealth platform that facilitates access to licensed clinicians for evaluation of metabolic health, weight care, and peptide programs. ELIVE RX does not provide emergency medical services. If you have a medical emergency, call 911 or go to the nearest emergency room.
Our website provides information about programs and pricing. It does not, by itself, constitute medical advice, diagnosis, or treatment.
3. Eligibility and Service Area
You must be at least 18 years old and located in a state or territory where our services are available at the time of care.
Telehealth services are available only in jurisdictions where AyaRx healthcare providers are licensed to practice medicine and where e-RxHub is authorized to dispense and ship prescribed medications.
4. No Guarantee of Prescription or Treatment
Payment for a program or package does not guarantee that a licensed clinician will prescribe medication or that any specific treatment will be recommended. Clinical decisions are made solely by licensed providers based on your health information, medical history, and applicable standards of care.
A prescription or medication must not be issued or dispensed prior to provision of care by a licensed medical professional, as required by applicable telemedicine laws.
5. Compounded Medications
Some medications offered through ELIVE RX programs may be compounded. Compounded medications are not FDA-approved and have not undergone FDA clinical studies to evaluate safety, efficacy, or quality. Your clinician will discuss risks and benefits when treatment is considered.
6. Pricing and Payment
Program prices displayed on our website are starting prices for direct-pay access. ELIVE RX does not accept insurance. Final pricing may depend on the program selected, clinical review, and fulfillment details. All payments are processed securely through Stripe.
7. Clinical Intake and Platform Services
After you select your medications and complete payment through Stripe, patient intake and health questionnaires are completed through Uniloop. Licensed clinicians review information submitted through Uniloop before any treatment recommendation is made.

Uniloop publishes its own Privacy Policy, Terms of Service, and HIPAA Compliance notices for its platform.
8. Clinical Model

9. Your Responsibilities
- Provide accurate and complete health information
- Use services only for yourself unless authorized as a caregiver
- Follow your clinician's instructions and report adverse effects promptly
- Keep your account credentials secure
10. Telehealth Consent
By using our telehealth services, you acknowledge that you have read and agree to our Telehealth Consent, including the benefits, limitations, and privacy considerations of receiving care remotely.
11. Intellectual Property
All content on this website, including text, graphics, logos, and design, is owned by ELIVE RX LLC or its licensors and protected by applicable intellectual property laws. You may not copy, modify, or distribute our content without written permission.
12. Limitation of Liability
To the fullest extent permitted by law, ELIVE RX LLC and its officers, employees, contractors, and partners shall not be liable for indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability for any claim shall not exceed the amount you paid to ELIVE RX for the program giving rise to the claim in the twelve months preceding the claim.
13. Dispute Resolution and Governing Law
These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles. Any dispute shall be resolved in the state or federal courts located in Harris County, Texas, unless otherwise required by applicable law.
14. Changes to Terms
We may update these Terms from time to time. Material changes will be posted on this page with an updated effective date. Continued use of our services after changes constitutes acceptance.
